Original report cards from the prior class that you passed. Original school departure or transfer certificates. Birth certificate (attested photocopy), medical clearance, and certification of good health. Twelve duplicates of the child’s most recent passport-size photos. Pictures and addresses of those who are authorized to meet the child(ren) and accompany them on weekends and vacations. A properly signed copy of the school’s rules and regulations.
